   Hi, Thanks so much for coming to our office today and giving us all that good info.
   I tryed the link for the tracker forms but it did not work…Help

     Hello Dalene,

     The download link shows where the optin form is located right after you fill it out. Go back to the page and see if you see a Blue link that says “Click Here to Download”.

     Please let me know of you still have issues and I will send it to you. 🙂
